ACCIDENTAL DEATH INSURANCE FOR COSTA RICA CARD HOLDERS

NATIONAL INSURANCE INSTITUTE (INS)

1. INSURANCE DESCRIPTION :
This Accidental Death Insurance Policy offers protection to each insured person in case of accidental death and re-patriation of mortal remains to his/her country of origin. The policy coverage applies exclusively while the insured is in Costa Rican territory.

2. COVERAGES AND INSURED AMOUNT:
The policy covers the following risks whenever the direct and only cause is an accident covered by the policy, occurred to you and your dependents
(if any) while you are in Costa Rican territory.

a. Accidental Death: The Institute will pay the total amount of $ US 10,000.
b. Repatriation of Mortal Remains: The Institute will refund expenses, with adequate proof of corresponding invoices, up to $ US 5,000.

The insurance policy offers for each policy holder insured, a maximum coverage of three (3) months from the issuance of the Costa Rica Card.

3. INSURED PERSONS
The insurance policy covers, without limits of age, the following persons who get the Costa Rica Card according to each card version (see Card Prices):
a. The insured
b. Spouse or partner ( if they are included in the family modality of Costa Rica Card )
c. Sons and daughters (if they are included in the family modality of Costa Rica Card), who have previously paid the corresponding premium.

4. INSURANCE PAYMENT
All requests for insurance payments should be carried out by Costa Rica Card, who files the claim with the insurance certificate and the claim letter. In addition, Costa Rica Card must submit the following documents to the Institute:

A. In case of Accidental Death:

1. Certificate of Death issued by The Civil Registry of Costa Rica or The Judicial Morgue, stating the cause of death.

2. Copy of the insured's identification (ID)

3. Any other requirement that the Institute determines advisable in order to analyze the case.

B. In case of Repatriation of Mortal Remains:
In addition to the requirements stated in point A (see above) Costa Rica Card should present the original invoices of the expenses incurred for the repatriation of mortal remains.
